Description
Chimes is hiring a Case Manager to provide community-based behavioral health services in Kent County. The role manages up to 30 clients, develops individualized service plans, helps consumers obtain housing, food, healthcare, employment, and other resources, monitors recovery-plan participation, provides crisis assessment and intervention, coordinates with families, payers, and service providers, and prepares documentation for re-authorizations and billing. The position is Monday through Friday from 9:00 a.m. to 5:00 p.m., pays $21.00 per hour, and requires an on-call rotation. Eligible candidates must meet one of several education, nursing, or experience criteria, obtain specified Pennsylvania and federal clearances, and may have additional related experience or education substituted at the Personnel Officer’s discretion.
