Description
The Case Manager will provide clinical case management services under the guidance of the Prince George's County Department of Corrections and the Rebirth Substance Use Treatment (RSUT) Program. Responsibilities include completing program enrollments, developing continuity of care plans, making referrals, following up with providers, submitting progress reports, maintaining HIPAA-compliant records, participating in clinical supervision, responding to inmate consultations, and attending training sessions. The role requires adherence to ethical standards and willingness to perform additional duties within the occupational scope.
