Description
Crossroads is hiring a full-time Case Manager to support the Clinical Coordinator in client services for a behavioral health and substance use treatment setting. The role develops and monitors individual treatment plans, manages a client caseload, documents progress, provides individual and group interventions, manages crises, coordinates care and aftercare, and assists clients with government assistance, court obligations, and referrals. The position requires a high school diploma or GED, 1–2 years of related experience or training, addiction and recovery knowledge, EMR and Microsoft Office proficiency, and current CPR/First Aid/Narcan training.
