Clinton Counseling Center is hiring a mid-level, contractual Case Manager/Supports Coordinator in Mt. Clemens, Michigan. The role supports individuals receiving outpatient substance abuse and mental health services through assessment, individualized service planning, coordination with providers and community resources, advocacy, documentation, case reviews, crisis intervention, and discharge planning. The position offers flexible hours, some local travel, and compensation to be determined.
