Description
The Case Manager provides psychosocial support, individualized care coordination, and resource linkage for clients with mental health and substance abuse needs. Responsibilities include assessing client needs, supporting recovery plans, assisting with daily living and medication adherence, coordinating with providers, maintaining client and family communication, documenting progress, attending supervision and multidisciplinary meetings, scheduling appointments, arranging transportation, and educating clients and families. The role requires a bachelor’s degree in social work, psychology, or a related field, preferably a master’s degree, relevant case management experience, strong communication and organizational skills, knowledge of community health resources, confidentiality, and a valid driver’s license with reliable transportation. Weekend, evening, or adjusted hours may be required.
