Description
The Case Manager coordinates hospital case management for patients, focusing on effective discharge planning to reduce recidivism and ensure continuity of care through follow-up appointments, medication management, housing, transportation, patient education, collateral information gathering, insurance coordination, crisis support, and interdisciplinary teamwork. The role requires strong documentation, communication, and clinical judgment in a psychiatric hospital setting, along with a degree in social services or BSW preferred, two years of psychiatric/mental health experience, CPR certification, and completion of Management of Assaultive Behavior training before assuming patient care responsibilities.
