Description
East Bay Innovations is hiring a full-time Case Manager I for its Transitional Care and Case Management department to support clients transitioning from nursing or long-term residential facilities into the community. The role coordinates housing, government benefits, IHSS, health services, and other community resources; maintains case records; builds support networks; and serves as a liaison among clients, families, landlords, employers, and agencies. The position supports clients with developmental disabilities, homelessness, substance misuse, mental health crises, hygiene challenges, and challenging behaviors, and requires two years of human-services experience, a bachelor’s degree or equivalent experience, a California driver’s license, a valid vehicle, and auto insurance. Compensation is $28.02 per hour, with medical, dental, vision, retirement, leave, training, and other benefits.
