Encompass Health is hiring a Case Manager II to coordinate patient care from admission through post-discharge in an inpatient rehabilitation setting. The role manages interdisciplinary treatment plans, discharge planning, resource coordination, documentation, utilization review, regulatory compliance, and patient and family communication. Candidates must be qualified to independently complete assessments within their discipline, hold an active license where required, and obtain CCM or ACM certification within two years of entry; nursing candidates need an associate degree and RN licensure, while other eligible health-care professionals need a bachelor’s degree.
