Description
Encompass Health is hiring a Case Manager II to coordinate patient care from admission through post-discharge in an inpatient rehabilitation setting. The role manages resources, develops and modifies interdisciplinary treatment plans, supports discharge planning, monitors patient experience and utilization, documents cases, makes referrals, and participates in utilization review and on-call activities. Candidates must be qualified to independently complete assessments within their discipline, hold an active license where required, meet CCM or ACM certification requirements, and have the specified nursing or other health-care professional education; rehabilitation experience is preferred.
