Emory Valley Center is hiring a full-time Case Manager to coordinate and oversee individualized care plans, assess needs, monitor progress, advocate for individuals, maintain case records, and collaborate with service providers and stakeholders. The role requires experience in case management, social services, or related fields, along with communication, organizational, problem-solving, and confidentiality skills. Preferred qualifications include a Tennessee Licensed Practical Nurse credential or an alternative education and experience combination, while DDA and managed-care training, First Aid, CPR, medication certification, and other required trainings are also specified.
