Description
The Case Manager leads the health, wellness, and care oversight of youth in a Community Residence, partnering with program staff and providing behavioral health overlay services, assessments, family support, home visits, service coordination, treatment-plan training, and documentation. The role requires a bachelor’s degree in human services, a master’s degree preferred, a valid driver’s license, and at least two years of experience working with children and families. The schedule is Tuesday through Friday from 1 p.m. to 11 p.m. and Saturday from 9 a.m. to 5 p.m., with on-call support, flexible hours, and generally 80% time at work.
