Description
HELP USA is hiring a Case Manager for its Philadelphia Rapid Rehousing program to provide housing-first supportive services to single young heads of household ages 18 to 24. The role conducts assessments, develops individualized service plans, connects clients to housing and community resources, facilitates support groups and workshops, coordinates with medical, mental health, and substance abuse providers, and assists with education and employment linkages. The Case Manager will also conduct monthly home visits and enter information in the AWARDS database. A bachelor's degree in social work or a related field is required, an MSW is preferred, and at least two years of case management, assessment, counseling, and crisis intervention experience is required.
