Description
The Case Manager coordinates client care, support, and case plans for individuals and families involved in Homefull Shelter, Housing, or Prevention programs. The role develops individualized service plans, maintains clinical and case records, supports housing stability and independent living, conducts home visits, community meetings, office-based work, and telehealth, and meets productivity and documentation goals. A bachelor’s degree in social work, counseling, psychology, or a related field is required, while experience with homelessness or marginalized communities is preferred.
