Description
The Case Manager coordinates client care, support, and case plans for individuals and families involved in Homefull Shelter, Housing, or Prevention programs. The role develops individualized service plans focused on housing stability, food access, income, benefits, and independent living; maintains clinical and case records; conducts home visits, community meetings, office-based services, and telehealth; and contributes to productivity and client-outcome goals. A bachelor’s degree in social work, counseling, psychology, or a related field is required, while experience with homelessness or marginalized communities is preferred.
