Horizon Services, Inc. is hiring a full-time Case Manager for its Diversion Triage Center in Pleasanton, California. The role provides case management and referral planning for clients with substance use, mental health, or co-occurring disorders; coordinates care, schedules appointments, arranges transportation, documents cases, provides crisis intervention, supports discharge planning, and collaborates with community providers and program staff. The position requires experience in mental health or substance use case management, crisis work, and diverse populations, with a bachelor’s degree in psychology or a related field and preferred master’s-level education. Compensation is $31 per hour, with a $2,000 sign-on bonus for bilingual applicants and medical, dental, life, and LTD benefits.
