Description

HousingPlus, a nonprofit organization dedicated to helping women overcome poverty, homelessness, addiction, trauma, and the effects of incarceration, is seeking a Full-Time Case Manager. This role involves providing direct support to women and women with children experiencing chronic homelessness, mental illness, substance use disorder, or a disabling medical condition. The Case Manager will manage a caseload, develop individualized service plans, conduct assessments, offer counseling, and collaborate with various service providers to achieve housing stability, family stabilization, independent living, and financial security.

Additional Information

Required Previous Experiences: Minimum of 2 years’ experience working with families around the issues of homelessness, Mental Illness and/or Substance Use Disorder.
Preferred Qualifications: English/Spanish speaking preferred, not required.
Experience working with veterans preferred, not required.
Familiarity with Human Resource Administration (HRA) policy and procedures.
Ability to work as a team member.
