Description
Highland Park CDC is hiring a full-time Case Manager to provide direct casework services to clients and residents. The role assesses client needs, develops and evaluates service and treatment plans, manages an assigned caseload, makes referrals, documents case activity, handles emergencies, participates in hearings and discharges, and attends supervision and professional development activities. The position is primarily on-site, generally Monday through Friday with possible rotating weekend coverage, and may require approximately 5% travel.
