Case Manager at I Am Boundless

I Am Boundless is hiring a community-based Case Manager to support individuals with intellectual and developmental disabilities and their families. The role focuses on behavioral health needs, service linkage, case management, tracking health outcomes, and coordinating with Boundless staff and outside providers. Applicants need experience with mental health conditions, a high school diploma or equivalent, a valid Ohio driver's license, auto insurance, an insurable driving record, and First Aid, CPR adult, child, and infant certification. A bachelor's degree in a related field or at least three years of human services experience, plus two years of case management or care coordination experience and experience with autism spectrum disorders and developmental disabilities, are preferred.
