Description
The Case Manager is the most senior level of direct-care staff in the Jewish Board’s supportive housing programs, working with adults with serious mental illness, substance use disorders, or trauma. The role carries complex clients and larger caseloads, develops person-centered service plans, supports medication management and independent living, advocates for residents, coordinates community services, documents care, manages crises, and assists with safety and wellness activities. The position requires a bachelor’s degree, at least one year of human services experience preferred, knowledge of health and human service systems, crisis de-escalation skills, and the ability to travel, drive, and lift 25 pounds.
