Description
The Case Manager provides direct care and case management to Deaf, Deafblind, and Hard of Hearing adults with serious and persistent mental illness in the Jewish Board’s supportive housing programs. The role carries complex clients, develops person-centered service plans, supports medication management and independent living, advocates for residents, coordinates community services, documents care, manages crises, and assists with safety and wellness activities. The position requires a BA or BS degree, fluent American Sign Language, and at least one year of Human Services experience; it also requires Microsoft Office familiarity, a valid driver’s license, and the ability to lift 25 pounds and manage stairs.
