Case Manager at Lifeline Connections

Lifeline Connections is hiring a Mental Health Case Manager to provide community-based behavioral health, care coordination, and re-entry support to participants. The role conducts assessments, develops and monitors individualized care plans, coordinates behavioral health, medical, housing, employment, and other community services, facilitates referrals, collaborates with multidisciplinary and external stakeholders, maintains regulatory-compliant documentation, and advocates for participant resources. The position requires a Washington State Department of Health credential, at least one year of relevant behavioral health experience, and knowledge of Washington State behavioral health regulations; a bachelor's degree in a related field is preferred. The salary is $22.62–$24.87 per hour, with medical, dental, and vision coverage and other benefits.
