Description
The Adult Case Manager provides community-based case management and care coordination for adults with serious mental illness, co-occurring disorders, and complex psychosocial needs. The role helps clients access medical, behavioral health, housing, financial, vocational, educational, and community resources; coordinates care across providers and systems; supports recovery and independence; and collaborates with interdisciplinary teams. Services are delivered in office, community, residential, hospital, and home-based settings, with requirements including a bachelor’s or relevant associate degree, multidisciplinary collaboration, strong communication and organizational skills, and the ability to travel and transport clients when appropriate.
