Description
The Case Manager at Odyssey House is responsible for providing counseling and case management services related to substance abuse, legal, mental health, medical and vocational/educational issues. They work with clients in individual and group counseling, advocating a drug-free lifestyle. The Case Manager formulates treatment plans, maintains clinical records, conducts education/prevention groups, and provides crisis intervention services. They also handle discharge planning, room inspections, and facilitate meetings, acting as a liaison with referral agencies and providing services to family members. Additional duties include client screening and participation in staff meetings and quality improvement activities.
