On Demand Counseling is hiring multiple full-time Case Managers for its Austintown team to support individuals with substance use and mental health disorders. The role connects clients with food, Medicaid, housing, transportation, and other community resources; coordinates referrals; supports discharge planning; advocates for clients; maintains electronic health record documentation; and monitors progress toward case management goals. Candidates must be at least 18, have a high school diploma or equivalent, pass background and drug screening, and have at least one year of case management experience.
