On Demand Counseling is hiring multiple full-time Case Managers for its Newton Falls team to support individuals with substance use and mental health disorders. The role connects clients with food, Medicaid, housing, transportation, and other community resources; coordinates referrals; supports discharge planning; advocates for clients; maintains electronic health record documentation; and monitors progress toward case management goals. Candidates must be at least 18, complete a background check and drug screen, hold a high school diploma or equivalent, and have at least one year of case management experience. The position offers weekday and weekend schedules, medical, vision, dental, supplemental insurance, life insurance, and a 401K.
