Description
Case Manager responsible for coordinating care and services for people with developmental disabilities. The role develops and facilitates Individual Service and Treatment Plans, coordinates providers and appointments, maintains client records, advocates for client safety and welfare, supports residential program implementation, handles crisis prevention and intervention, provides emergency coverage and on-call support, and completes required documentation while maintaining HIPAA and professional confidentiality.
