Description
The Licensed Programs Case Manager coordinates and manages person-centered services for approximately 40 participants, develops and monitors Person-Centered Plans and Individual Service Plans, maintains regulatory records, supports interdisciplinary teams, provides advocacy and resource coordination, and covers participant needs and program operations. The role also oversees program implementation, assists with staff training, transports participants when applicable, and requires compliance with developmental disabilities, community care licensing, and regional center standards. It is a full-time, Monday-through-Friday position reporting to the Program Manager, with work in indoor and community-based environments.
