Description
The Case Manager supports behavioral health members through treatment planning, crisis management, direct oversight, goal attainment, community service navigation, discharge follow-up, and daily living support. The role requires collaboration with multidisciplinary teams, documentation, family and member contacts, and on-call emergency coverage, with physical requirements including lifting up to 20 pounds and training in crisis de-escalation and restraint methods. Qualifications include a high school diploma or associate degree with relevant experience, a valid non-commercial driver's license, and strong communication and organizational skills.
