Description
The Case Manager I, II, III supports behavioral health members through treatment planning, crisis management, direct case management, group facilitation, community service navigation, discharge follow-up, and daily living support. The role requires collaboration with multidisciplinary teams, documentation, on-call coverage, and physical activities including lifting up to 20 pounds and applying restraint techniques when trained. Qualifications include a high school diploma or associate degree with specified experience in mental health, behavioral health, or drug and alcohol services, a valid non-commercial driver's license, and strong communication and organizational skills.
