Description
The Case Manager supports behavioral health members through treatment planning, crisis management, direct oversight, multidisciplinary collaboration, community service navigation, discharge follow-up, and documentation. The role requires a high school diploma or associate degree with specified experience in mental health, behavioral health, or drug and alcohol services, a valid non-commercial driver's license, strong communication and organizational skills, and the ability to perform physically demanding tasks including crisis de-escalation and restraint techniques.
