Description
The Case Manager (RN) provides individualized patient assessment, care coordination, and transition planning for hospitalized patients in the Integrated Care Management Program at Miami Valley Hospital North. The role coordinates with physicians, payors, families, and interdisciplinary teams; supports discharge planning and resource allocation; and contributes to clinical outcomes and efficient use of healthcare resources. It is a full-time 8:30 a.m. to 5 p.m. position requiring a BSN, Ohio RN licensure, and at least three years of relevant clinical experience in the past five years.
