Description
The Case Manager is a registered nurse responsible for individualized patient assessment, care coordination, and transition planning to improve length of stay, resource use, and care outcomes. The role coordinates with physicians, payors, families, and interdisciplinary teams, and requires expertise in acute care nursing, healthcare reimbursement, clinical outcome analysis, utilization management, transition planning, resource allocation, and team communication. The position is full-time, 80 hours per pay period, with a 0830–1700 shift at Miami Valley Hospital.
