Project 180 is hiring a Project Based Housing Case Manager to provide intensive, wraparound, on-site and field-based case management for high-need, high-barrier behavioral health clients in the Los Angeles County jail system. The role includes client outreach, custody pickups, court accompaniment, needs assessments, resource linkage, recovery-skills teaching, multidisciplinary care coordination, documentation, and billing. It requires a bachelor’s degree or two years of case management experience, at least one year of case management experience, experience with culturally diverse populations, a valid California driver’s license, proof of insurance, and an own vehicle. The position is supervised by a Program Supervisor, requires regular attendance, and may involve holiday or weekend work.
