Description
The Case Manager assesses participant needs, develops and coordinates individualized service treatment plans, provides mobile community-based support, advocates for participants, builds community networks, and documents care in compliance with agency, funder, and licensing standards. The role requires a bachelor’s degree in Human Services or a related field, at least two years of direct support experience in behavioral health, addiction recovery, or IDD services, a valid driver’s license, and intermediate computer skills. It is primarily field-based, requires driving participants to appointments, and involves frequent sitting, standing, walking, bending, reaching, and lifting up to 50 pounds.
