Description
RHD is hiring a Case Manager to provide community-based case management, advocacy, service coordination, documentation, and support for individuals and families. The role works with participants, families, clinical staff, medical providers, and community partners to develop recovery plans, connect people with behavioral health, healthcare, housing, employment, educational, vocational, and other services, monitor progress, and navigate complex service systems. A bachelor's degree in a related field is required, while one year of case management experience and two years of experience in behavioral health, substance use recovery, intellectual and developmental disabilities, or human services are preferred. The position requires a valid driver's license, ability to travel throughout the service area, intermediate computer skills, and the ability to assist individuals as needed.
