Description
The Case Manager provides therapeutic case management for adolescents and families, coordinating medical care, Individual Service Plans, treatment goals, appointments, medication administration, documentation, and multidisciplinary team collaboration. The role is full-time, Monday through Friday in Germantown, Pennsylvania, and requires a bachelor's degree, strong communication and documentation skills, critical thinking, and completion of required certifications. Medical, dental, and vision insurance are available after 30 days of employment.
