Description
The Case Manager assesses participant needs, coordinates community-based services and resources, develops and reviews recovery plans, advocates for participants, maintains confidential documentation, and provides crisis intervention. The role requires a bachelor’s degree in a relevant human services field, at least two years of relevant experience, computer proficiency, a valid driver’s license, and the ability to pass background checks. It involves frequent community travel, participant-home and agency visits, and a rotating 24-hour on-call schedule.
