Description
The Targeted Case Manager provides case management services for individuals with intellectual and developmental disabilities, including intake, assessment, crisis intervention, advocacy, referrals, family monitoring, home visits, care coordination, and development of individualized plans of care. The role requires a bachelor’s degree in human services or equivalent education and experience, one year of related work experience, a valid driver’s license, and current CPR/First Aid certification. Full-time, part-time, and as-needed schedules are available, with a pay range of $20.00 to $25.00 per hour.
