Description
The Case Manager provides housing-first and harm-reduction support to people experiencing long-term homelessness, helping them achieve housing stability, access community services, establish government benefits, improve financial health, and connect with education, training, and employment. The role maintains a caseload of at least 18 participants, completes intakes and documentation, conducts home visits, coordinates with property managers and community resources, and provides transportation and flexible support. It is an entry-level full-time position paying $23.00–$25.00, requiring a bachelor’s degree in human services or equivalent experience, mental health and chemical health experience, a driver’s license, and a car.
