Description
The Case Manager will provide case management and housing-support services to formerly homeless adults at Aeon’s OHP properties in Downtown Minneapolis. The role maintains a caseload of at least 18 participants, completes intakes and tenancy applications, conducts home visits, connects residents with community resources, supports housing and financial stability, assists with benefits and employment, and maintains case documentation. It is a full-time position paying $23–$25 per hour, with a Monday-to-Friday schedule and flexible hours, and requires a bachelor’s degree in human services or equivalent experience, a driver’s license, and a car.
