Description
Team Select Home Care is seeking a registered nurse Case Manager to oversee assigned home-health patients and ancillary staff, develop and evaluate physician-directed care plans, conduct patient assessments, coordinate multidisciplinary services, administer medications, educate patients and caregivers, maintain clinical documentation, and support quality assurance and on-call nursing coverage. The role requires a current state nursing license, at least one year of nursing experience, strong communication and time-management skills, computer proficiency, transportation for required travel, and current CPR/health clearances. The position is exempt, pays $70,000–$85,000 annually with bonus, and includes medical, dental, vision, paid time off, paid sick time, 401(k), and referral benefits.
