Description
The full-time Case Manager works in a community setting to access, coordinate, and monitor services for clients recovering from alcoholism or substance abuse. The role assesses client needs, develops and monitors treatment plans, coordinates with community agencies and insurance providers, advocates for clients, supports basic and specialized needs, and maintains documentation and confidentiality. It requires a bachelor's degree in social work, psychology, or a related field, a valid driver's license, reliable transportation, current automobile insurance, and completion of TCM Core Training and SUD training within six months of hire.
