Description
The Case Manager coordinates daily schedules and treatment-related activities for youth residents at The Centers’ H.O.P.E. Campus, serves as a liaison with child welfare agencies, schools, families, and other stakeholders, and manages appointments, visits, transportation, documentation, case reviews, treatment and discharge plans, crisis de-escalation, and Epic billing. The role requires a bachelor’s degree in social work, counseling, or a related field and at least two years of experience in counseling, childcare, social work, or therapeutic support services for children and families.
