Description
The Case Manager provides case management, crisis intervention, discharge planning, group and individual work, mental health and substance use disorder counseling, screening, referrals, coordination with physical health providers, placement support, transportation, and field-based services. The role requires accurate documentation, confidentiality, program participation, and shift coverage, including part of the on-call team. Qualifications include a master's degree or a bachelor's degree with four years of social-services experience, a valid California driver's license and current auto insurance, cultural competency, and preferred experience with mental health or substance use disorder services, bilingual ability, and knowledge of Microsoft Office and EHR software.
