Description
The Case Manager provides case management, crisis intervention, discharge planning, group and individual work, mental health and substance use disorder counseling, screening, referrals, coordination with physical health providers, placement support, transportation, and field-based services. The role also requires accurate documentation, confidentiality, program participation, and shift coverage, including part of the on-call team. Qualifications include a master's degree or a bachelor's degree with four years of social-services experience, cultural competency, a valid California driver's license and auto insurance, and preferred experience with mental health or substance use services, bilingual ability, Microsoft Office, and EHR software.
