Description
The Case Manager provides case management, crisis intervention, discharge planning, group and individual work, mental health and substance use disorder counseling, screening, referrals, coordination with physical health providers, placement support, transportation, and field-based services. The role requires a bachelor's degree, two years of social-services experience, a valid California driver's license, current auto insurance, cultural competency, and confidentiality; mental health or substance-use experience, bilingual ability, and Microsoft Office or EHR knowledge are preferred.
