Description
The Case Manager provides comprehensive case management for clients, including crisis intervention, discharge planning, individual and group work, mental health and substance use disorder counseling, screening, referrals, documentation, placement support, transportation, field services, and coordinated care. The role also provides onsite and school-based services to youth in the MHSSA Program, participates in program meetings, and may assist with community trainings. Qualifications include a bachelor's degree, one year of social-services experience, cultural competency, a valid California driver's license, and current auto insurance; SUD certification, bilingual ability, Microsoft Office, and EHR software knowledge are preferred or required depending on the service area.
