Description
Unison Health is hiring a Case Manager to provide direct support to youth and families in the Toledo area. The role coordinates services, develops treatment plans, delivers therapeutic behavioral services, documents care, maintains confidentiality, and works with community partners in clients’ homes, the community, and group settings. The position requires a high school diploma or GED with relevant work experience, knowledge of Lucas County behavioral health services, and the ability to manage crisis situations and build therapeutic relationships; an associate or bachelor’s degree in a related field is preferred.
