Description
The Case Manager, Permanent Supportive Housing Program (PSHP) will assess and plan services for homeless clients, coordinate housing, health, employment, mental health, substance abuse, financial, and other supportive services, maintain case records and referrals, liaise with property management, and assist with housing placement and relocation. The role requires a bachelor's degree in a relevant social service or healthcare discipline and two years of case management experience, with a master's degree preferred. The position pays $60,000-$65,000 annually, is open until filled, and may work remotely after six months or as needed.
