Urban Pathways is hiring a Case Manager to provide clinical and case management services for assigned clients, including housing, employment, education, health, crisis intervention, advocacy, counseling, fieldwork, home visits, and client outreach. The role reports to the Director of Social Services or Social Services Supervisor, requires a bachelor’s degree, offers a hybrid schedule of four office days and one remote day, and includes medical, dental, and vision insurance among other benefits.
